Our Staff is what defines the Community Health Institute, comprised of a creative, dedicated and competent team, and characterized best by our passionate commitment to the needs of clients.

As residents of Northern New England, we have a deep understanding of the area in which we live and work. We also have the ability to draw upon the resources of a larger team of consultants from our parent organization, JSI Research and Training Institute, Inc (JSI). Collectively, our team comprises experts in nearly all areas of the health care system, which allows us to respond to the particular needs of our clients and those we serve.

Each of our employees bring years of expertise in their field. CHI, along with JSI, is organized in a flexible and dynamic way that allows the best team and leadership configuration for your project. Alyson Cobb
Alyson Cobb, BA, is a volunteer with the NH Red Cross AmeriCorps*VISTA Project serving at CHI. She is working to build New Hampshire’s capacity to respond to disasters and be prepared for emergencies through development of NH’s Medical Reserve Corps. Her work with the MRC units includes ensuring the readiness of the 15 units to respond to emergencies, building and implementing a state-wide recruitment plan, developing a volunteer intake system, and helping to organize an annual MRC training calendar. Amy Cullum
Amy Cullum, MA, MPH, is a senior consultant with CHI. She works with communities to assess and strengthen the public health infrastructure and public health emergency response capacity. Her areas of expertise include program planning and evaluation, quality improvement, and data analysis. Prior to coming to New Hampshire, Amy worked in Asia and Africa on projects to strengthen primary health care delivery systems. Arielle Slam
Arielle Slam, BA, BS, provides project support for a CHI project providing technical assistance in the implementation and evaluation of evidence-based interventions. She also provides coordination for a statewide learning collaborative in substance abuse prevention. Arielle’s expertise includes budget management, proofreading and formatting, and research. She earned a BS in Mass Communications and a BA in International Relations from Boston University. Arielle is interested in health economics and spent time researching and observing health systems in developing countries. Buffi Girardin
Buffi Girardin,Graphic Designer, has 19 years experience in graphic design and project management. She works extensively with Adobe InDesign, PageMaker, Corel Draw, Illustrator, Photoshop and Quark. Her print work includes: brochures, posters, print ads, annual reports, handbooks, logos, menus, presentations and package design. Debra Love
Debra Love, BS, provides project support for the CHI and has many years of experience in administrative management, conference logistics, database/website maintenance, proposal proofreading/editing, and brochure and newsletter design. Debbie earned her BS in Interdisciplinary Studies from Plymouth State College with emphasis in Business, Psychology & Health. She also received a writing degree from the Institute of Children's Literature, a certification in Medical Transcription and is a published poet and Notary Public. Debra’s professional interests include expanding her education in the area of graphic design and continuing to work on projects focused on the health of our youth with regard to substance abuse, alcoholism and obesity. Diane Lewis
Diane Lewis, BS, has been a project manager, data, and mapping analyst with the CHI since 2000. Her areas of expertise include project coordination and maintenance, information system and software testing, development and analysis of relational mapping and databases, extensive map and cartographic creation, and technical assistance. She earned her BS in Occupational Safety from Keene State College with a minor in Business Management. Diane enjoys working with community health centers and advising on system improvements. Dorothy A. Bazos
Dotty Bazos, RN, MS, PhD, is a registered nurse and holds a PhD from Dartmouth College in Health Policy and Clinical Evaluative Sciences. She has 15 years experience working in health policy and planning in NH, and previous to NH, in communities in third world countries. In her capacity as Director of Community Development at the CHI., Dotty collaborates with physicians and health providers, leaders of health care systems and policy analysts, nationally and in New Hampshire, to develop and evaluate programs and tools to address the critical issues of cost, quality, and access of local health care systems. Eric S. Turer
Eric Turer, MBA, is a senior consultant with the CHI. He has experience as a project manager, a skilled analyst, and an expert in issues pertaining to underserved populations, including rural health care delivery systems, community health centers, and integrated delivery systems since 1994. He has worked extensively with providers in underserved areas throughout the country and conducted extensive data analysis in support of community based planning, health care needs assessment, program evaluation, and policy development. Jonathan Stewart
Jonathan Stewart, MA, MHA, is the Director of the CHI. He has provided technical assistance, training, policy analysis and program evaluation services to state and community-based organizations in New Hampshire and the New England region since 1986. His expertise focuses on development of sustainable public health and rural health delivery systems. His previous work includes the start-up and management of a rural Community Health Center network in Northern New Hampshire, Ammonoosic Community Health Services, as well as, serving as the founding Director of the North Country Health Consortium. He served as co-chair of the Healthy New Hampshire 2010 initiative and Treasurer of the New Hampshire Public Health Association. Karen Forest
Karen Forest, BA is the Administrative Coordinator at CHI. She works with the Operations Director to enhance office systems to increase overall effectiveness and productivity. She also serves as the point person for troubleshooting office IT issues. She holds a BA in International Relations, Political Science and a minor in French from Boston University. Karen’s experience includes building sustainable health care systems in sub-Saharan Africa, with a focus on training African healthcare works in Infectious Disease specialization care. Karen Paddleford
Karen Paddleford, BA, is a project assistant with the CHI and supports a major initiative to collect ‘Uniform Data System’ (UDS) information from all Bureau of Primary Health Care grantees, Indian Health Services grantees, and National Health Service Corp non-federally funded sites with provider placements across the country. In working on this project, Karen provides technical assistance and support to providers and grantees from around the country. Additionally she is involved in extensive analysis and management of reported data. Karen's interests extend to the public health areas of environmental health and mental health. Karyn Madore
Karyn Madore, MEd, is the Operations Director and a senior consultant with the CHI. She has 12 years experience in the areas of women, adolescents and children's health & safety issues. She is an experienced coordinator of state-wide media campaigns and community-grant programs, as well as, all aspects of program management including researching efficacy, resource coordination, and implementation. Katie Robert
Katie Robert, BA, MPA, is a project assistant with the CHI. She provides technical support to federally funded grantees and sites with reporting questions and software issues for several federal uniform data system collection projects. She performs budget management tasks for several projects, and also has experience analyzing survey data and presenting that data in reports. Katie received her MPA and her BA in Political Science from the University of New Hampshire. Her interests include policy development, social marketing, and survey design and assessment. Lea R. Ayers LaFave
Lea Ayers LaFave, RN, PhD, is a senior project director with the CHI. She provides consultation in the areas of quality improvement, learning collaboratives, community health assessment, continuity of operations planning for organizations providing services to vulnerable populations, and program assessment and evaluation. Lea has experience as a nurse-educator in community health nursing and nursing leadership, and serves on the Goverment Affairs Commission for the New Hampshire Nurses' Association. She earned her BA in Psychology from Smith College, her Master's in Nursing from Yale University School of Nursing in Nursing Management and Policy in Community Health, and PhD from the University of Massachusetts Graduate School of Nursing at Worcester. Lea has broad experience as a registered nurse working across the healthcare system and the continuum of care, and has a particular interest in improving the linkages between healthcare and public health. Lisa Muré
Lisa Muré, MEd, is a senior consultant with the CHI. She is a former non-profit director and a former state administrator for alcohol, tobacco and other drug prevention services. She has extensive experience in coalition development, strategy development, best practice implementation, program evaluation, strategic planning and public and private partnerships. Lisa earned an M.Ed. in secondary education from Notre Dame College and a BA in Psychology from the University of Wisconsin - Madison. Lori Walter
Lori Walter, CMP, is a project assistant with the CHI and provides support in the development of public health emergency preparedness and substance abuse treatment programs. Lori is a Certified Meeting Professional (CMP) through the Convention Industry Council and manages the logistics of major statewide conferences for the CHI. Additionally, she attended NH Technical Institute with concentrated coursework in hospitality sales, convention planning, marketing, and hotel management. Martha Bradley
Martha Bradley, MS, is a senior consultant with the CHI. She has offered experience in the public health and health promotion field since 1991. Her expertise includes design of statewide strategies for education, outreach and promotion, along with coordination of community health education programs. Patricia DiPadova
Paddy DiPadova, MBA, is a senior consultant with the CHI. She has over 20 years experience in practice and contract management, HIPAA compliance, health center operations, network development and managed care contracting. Priscilla Davis
Priscilla Davis, BS, is a project manager with the CHI. Her responsibilities involve extensive management of nationally reported data, as well as, the development of unique software to collect, manage, and screen data electronically. Her expertise also includes training, editing, survey analysis, database creation and report management, and coordination of large-scale conferences. Rachel Kohn
Rachel Kohn, MSW, MPH, is a Senior Consultant with the CHI. Her areas of technical expertise include: project management; needs assessment; program evaluation; survey research design and administration; technical assistance for program-level and client-level program planning and implementation; all aspects of qualitative and quantitative data collection and management; grant writing, and report writing and dissemination. These technical skills are complimented by over three years of graphic design experience which is applied toward the visual presentation of data and health communication strategies. Ms. Kohn earned her BA from the University of Massachusetts, her MPH from Boston University School of Public Health, and her MSW from Boston University School of Social Work. Shasta Jorgensen
Shasta Jorgensen, BA, is a project coordinator for the CHI and has provided support for numerous projects since 2003. Through these projects, she has advanced her knowledge and skills around project management, data collection, and grant writing. Shasta has experience working in the areas of chronic diseases (primarily tobacco prevention and cessation), adolescent health and quality improvement. She earned her BA from University of Redlands in Redlands, California with a major in International Relations. Shasta is currently completing her Master of Public Health degree at Boston University School of Public Health with a Social and Behavioral Science concentration to enhance her strong interest in prevention of chronic diseases, evaluation and quality assurance. Susan Friedrich
Suz Friedrich, MBA, is a Managing Director with JSI and senior consultant with the CHI. She has provided consultation in the areas of health services delivery, public health, practice management, information for decision making, and program evaluation since 1985. She is an expert facilitator and trainer. Clients include government agencies, and public and private health care providers. She earned her BA from Amherst College and her MBA with honors from Boston University's School of Management in Health Care Administration.
